Each student participating in sports and/or activities/clubs/organizations must fill out all 6 forms below.Students participating in sports and activities/clubs/organizations do NOT have to fill out the forms twice.

6. 2023-24 Activities Code Video Acknowledgement Form 

Students participating in sports, must have a current physical on file at school. For the 2023-24 school year, only physicals dated on or after April 1st, 2022 will be considered valid. Students MUST have a current physical on record at school PRIOR to the first day of practice. 

 

 

Students who have a current physical on record at school will need to fill out an alternate year card. You can print one at the link below or coaches will send one home with athletes the first day of practice.
